India has ramped up its commitment to eliminate viral hepatitis as a major public health concern by 2030, aligning with the World Health Organization's global targets. This ambitious initiative addresses a disease burden that affects millions of Indians, many of whom remain unaware of their infection status.

Hepatitis, an inflammation of the liver caused primarily by five distinct viruses (A, B, C, D, and E), poses a significant health challenge in India. The country accounts for a substantial portion of the global hepatitis burden, with an estimated 40 million people living with chronic hepatitis B and 6-12 million with hepatitis C. Without proper treatment, these infections can lead to cirrhosis, liver failure, and liver cancer.

The National Viral Hepatitis Control Program

India's renewed efforts build upon the National Viral Hepatitis Control Program launched in 2018. The program focuses on prevention, screening, and providing affordable treatment to those affected. Key components include establishing testing facilities at primary, secondary, and tertiary healthcare levels, ensuring the availability of diagnostic services even in remote areas.

The government has made significant strides in making hepatitis treatment more accessible and affordable. Anti-viral medications for hepatitis C, which once cost thousands of dollars, are now available at drastically reduced prices through government procurement and generic manufacturing. For hepatitis B, tenofovir and entecavir are provided free of cost at government facilities to eligible patients.

Expanding Vaccination Coverage

Prevention through vaccination remains a cornerstone of the elimination strategy. Hepatitis B vaccination has been included in India's Universal Immunization Program since 2002, with birth dose vaccination introduced in 2011. The government is working to achieve universal coverage of the birth dose, which is crucial for preventing mother-to-child transmission.

Public awareness campaigns aim to educate communities about transmission routes, prevention methods, and the importance of testing. These campaigns specifically target high-risk populations including pregnant women, people who inject drugs, healthcare workers, and individuals with multiple sexual partners.

Integration with Existing Health Programs

The hepatitis elimination strategy has been integrated with other national health programs for greater efficiency. Testing and treatment services are being linked with programs for HIV/AIDS, tuberculosis, and non-communicable diseases. This integrated approach allows for comprehensive care and reduces the stigma often associated with hepatitis testing.

Blood safety measures have also been strengthened, with mandatory screening of all donated blood for hepatitis B and C. This protects recipients while also serving as a case-finding opportunity.

Challenges Ahead

Despite progress, several obstacles remain. Rural areas still face shortages of diagnostic facilities and trained healthcare workers. Awareness levels remain low in many communities, and stigma continues to prevent people from seeking testing and treatment. The COVID-19 pandemic disrupted routine immunization and screening services, creating gaps that need to be addressed.

Additionally, hepatitis E, which spreads through contaminated water, remains a concern in areas with poor sanitation. Addressing this requires improvements in water supply and sanitation infrastructure alongside medical interventions.

Meeting the 2030 Target

The WHO's 2030 elimination targets call for a 90% reduction in new chronic infections, 90% diagnosis of existing infections, 80% of eligible patients receiving treatment, and a 65% reduction in mortality. Achieving these goals will require sustained political commitment, adequate funding, strong surveillance systems, and continued collaboration between government agencies, healthcare providers, and civil society organizations.

India's intensified efforts demonstrate recognition that hepatitis elimination is achievable with the right combination of prevention, testing, and treatment strategies. Success will not only save millions of lives but also reduce the economic burden of chronic liver disease on families and the healthcare system.
